What Is Premature Ejaculation?

Premature ejaculation (PE) is ejaculation that occurs sooner than desired, either before or shortly after penetration, causing distress to either partner. Clinically: ejaculation within approximately 1 minute of penetration consistently. But practically, what matters is whether both partners are satisfied — not a stopwatch.

Prevalence: 20–30% of men report PE. It is the most common male sexual complaint, yet one of the least discussed in Pakistan due to shame around the topic.

Causes

Psychological

  • Performance anxiety
  • Excitement and arousal mismanagement
  • Early sexual experiences where speed was necessary (secretive, rushed encounters)

Biological

  • Hypersensitivity of the glans
  • Low serotonin levels (serotonin delays ejaculation)

Treatments — Highly Effective

Behavioural Techniques (First-Line)

Start-stop technique: Stimulation is stopped just before the point of ejaculatory inevitability, then resumed after the urge passes. Repeated 3–4 times before allowing ejaculation. Trains the nervous system to tolerate higher arousal without ejaculating. 60–90% success rate with consistent practice.

Squeeze technique: At the point of near-inevitability, squeeze the frenulum area firmly for 10–20 seconds. Reduces arousal below the threshold. Can be performed by either partner.

Sensate focus exercises: Graduated intimacy exercises that rebuild sexual confidence without the pressure of performance. Typically done in a course of psychosexual therapy.
