Hormones and Sexual Health — Complete Guide

Your hormones run your intimate life. Here is the complete picture — which hormones matter, what happens when they are off, and how to restore balance naturally and medically.

The Key Hormones for Intimate Health

Testosterone — The Desire Hormone (Men AND Women)

Testosterone drives libido in both sexes. In men: produced primarily in the testes (95%) and adrenal glands (5%). In women: produced in the ovaries and adrenal glands in much smaller amounts — but these small amounts are critical.

Signs of low testosterone (men): Low libido, erectile difficulties, fatigue, decreased muscle mass, increased body fat, brain fog, depression, reduced morning erections.

Signs of low testosterone (women): Low libido, reduced sensitivity, fatigue, difficulty building muscle, low mood.

Optimising testosterone naturally: Resistance exercise, adequate sleep, zinc supplementation, vitamin D, reducing body fat, managing stress, and avoiding pornography (dopamine dysregulation affects testosterone signalling).

Oestrogen — The Female Foundation

Oestrogen maintains vaginal tissue health, lubrication, sensitivity, and genital blood flow. It also contributes significantly to mood, bone health, and cardiovascular health.

Signs of low oestrogen (women): Vaginal dryness, painful intercourse, reduced lubrication and sensitivity, hot flashes, mood changes, brain fog, dry skin.

Low oestrogen causes: Menopause, breastfeeding, oral contraceptives (some), extreme exercise or low body fat, eating disorders.

Oxytocin — The Bonding Hormone

Released during physical touch, orgasm, breastfeeding, and social bonding. Increases trust, reduces cortisol, deepens attachment. Called "the love hormone" in research literature.

Increasing oxytocin naturally: More physical affection with your spouse (hugging, holding hands, massages, kissing), eye contact, sexual intimacy, shared positive experiences, and prayer together.

Cortisol — The Enemy of Intimacy

As detailed in the stress page: cortisol directly suppresses all reproductive hormones. Chronic elevated cortisol is one of the most common and overlooked causes of low libido, sexual dysfunction, and relationship deterioration in Pakistan.

Thyroid Hormones

Thyroid hormones regulate metabolic rate — when they are off, everything is off. Hypothyroidism (low thyroid) causes fatigue, weight gain, low libido, depression, and constipation. Affects 1 in 8 women. A TSH blood test is inexpensive and highly informative.

Thyroid and intimate health: Untreated hypothyroidism is a common and often missed cause of low libido in Pakistani women. It is also treatable with a daily tablet.

Hormone Testing in Pakistan

A comprehensive hormone panel — available at most reputable laboratories in Karachi, Lahore, and Islamabad — should include: testosterone (total and free), oestradiol, progesterone, LH, FSH, prolactin, TSH, free T4, cortisol (morning), DHEA-S, SHBG, and fasting insulin. Cost: PKR 8,000–15,000 depending on tests selected. Worth every rupee to understand your hormonal baseline.
