Painful Sex in Women — Causes and Solutions

Painful sex is not normal, not inevitable, and not permanent. It is also extremely common — affecting up to 1 in 5 women at some point. This guide explains why it happens and what to do.

Pain Is Not Normal — But It Is Common

Many Pakistani women are told — explicitly or implicitly — that sex is supposed to hurt, especially at first. This is wrong. Occasional discomfort during adjustment is one thing; consistent pain is a symptom requiring attention.

Common Causes

Insufficient Arousal and Lubrication

The most common and most fixable cause. The vagina lubricates when aroused. If intercourse begins before adequate arousal, the friction causes pain. Solution: more foreplay, more time, communication.

Vaginismus

Involuntary vaginal muscle contraction. Can make penetration painful or impossible. Completely treatable — see gynaecologist who can refer to pelvic floor physiotherapy.

Infections

Endometriosis

Tissue similar to the uterine lining grows outside the uterus. Causes deep pelvic pain during sex, menstrual pain, and sometimes infertility. Affects ~10% of women. Requires gynaecological diagnosis and management.

Hormonal Changes

After childbirth, during breastfeeding, and approaching menopause — oestrogen decreases cause vaginal dryness and tissue changes. Lubricants and medical treatment available.

Psychological Causes

Anxiety, history of sexual trauma, relationship tension — all can manifest as physical pain through muscle tension. Psychological treatment (CBT, trauma therapy) can resolve physical symptoms.

See a gynaecologist if pain persists. Do not simply endure. Every Pakistani city has gynaecologists — request a female doctor if more comfortable.
